The Agency’s mission is to provide paraprofessional services to clients in their place of residence thereby assisting them to realize his or her highest level of independence and quality of life. We are committed to providing quality care/service by staff members who recognize the value of the aged and disabled.

Philosophy
Our goals and services are based on two fundamental philosophical principles: the belief in the innate worth of the aged and disabled individual and the belief that each individual, regardless of age, race, color, creed, sex, national origin or handicap(s) is entitled to maximize his potential as a human being and as a member of society.
It is the contention of this agency that the aging process is a normal state in the development of any individual and that chronic disease and disability are, to some degree, a part of that process. This Agency is dedicated to supporting aged and disabled individuals within the confines of their residence, in order that they may maximize their contributions and fulfill their goals as a family member and member of society with a minimum of conflict. In accomplishing this end, it is felt that the self-respect of the individual can and will be enhanced.
Our Agency is also a vital member of its community. As an employer, the Agency practices non-discrimination and strives to provide opportunities for personal and professional growth. As an integral part of the business community, the Agency makes every effort to serve the people with attention to current area practices and specific needs.
AGENCY GOALS
To provide a broad base of services within the scope, allowing clients to be involved in establishing, implementing, and evaluating services.
To support clients in their place of residence as long as possible and provide services in the least restrictive setting.
To provide safe, cost-competitive quality services.
To develop a relationship with referral sources to effectively meet our clients’ home needs.
To function as a liaison between our clients and available community resources and assist our clients in addressing their needs and concerns.
Agency Purpose
We believe home services are an essential part of continuing care/service to clients in their place of residence, home services minimize the effects of illness, accident, or disability.
Agency recognizes the intrinsic worth of aged, diseased and/or disabled persons. We believe the contributions of these persons are no less important than the contributions of other members of our society. Agency recognizes that these persons are entitled to retain or regain their maximum participation in society.
We recognize that the aging process is an unavoidable part of life. We understand chronic disease and disability may be part of that process.
In providing care/services to individuals, the Agency recognizes that clients and families have inalienable rights to self-determination. They have the right to participate in planning for their care/service, to make decisions about their home care/service, and to maintain their independence as long as feasible.
The agency recognizes that staff behavior, attitudes, knowledge, and understanding have an important influence on helping clients.
Therefore, we are committed to assisting all individuals within the structure of their home environments, allowing maximum contribution, independence, and retention of self-respect.

Client Rights and Responsibilities
It is anticipated that observance of these rights and responsibilities will contribute to more effective care and greater satisfaction for the Client as well as the staff. The rights will be respected by all personnel and integrated into all Home Care programs. A copy of these rights will be given to Clients and their families or designated representative. The client or his/her designated representative has the right to exercise these rights. In the case of a client adjudged incompetent, the rights of the client are exercised by the person appointed by law to act on the client's behalf. In the case of a client who has not been adjudged incompetent. Any legal representative may exercise the client's rights to the extent permitted by law.
1. To be fully informed and knowledgeable of all rights and responsibilities before providing preplanned care and to understand that these rights can be exercised at any time.
2. To choose a health care provider.
3. To request services from the Home Care Agency of their choice and to request full information from their agency before care is given concerning services provided, alternatives available, licensure and accreditation requirements, organization ownership and control.
4. To be informed in advance about care to be furnished and of any changes in the care to be furnished before the change is made.
5. To be informed of the disciplines that will furnish care and the frequency of visits proposed to be furnished and to know that all staff is properly trained and competent to perform their duties.
6. To information necessary to give informed consent prior to the start of any procedure or treatment and any changes to be made.
7. To participate in the development and periodic revision of the Service Plan. 8. Confidentiality and privacy of all information contained in the client record and of Protected Health Information.
9. To information necessary to refuse care within the confines of the law and to be informed of the consequences.
10.To treatment with utmost dignity and respect by all agency representatives, regardless of the client's chosen lifestyle, marital status, cultural mores, political, religious, ethical beliefs, whether or not an advance directive has been executed and source of payment without regard to race, creed, color, sex, age or handicap.
11.To have his/her property and person treated with respect, consideration and recognition of client dignity and individually.
12.To receive and access services consistently and in a timely manner from the agency to his/her request for service.
